Find Your Tax Forms
Can’t find your Form 1095-A or Form FTB 3895? Sign in to your Covered California account or use our chatbot CiCi to download your tax forms.
You get your tax forms the same way you chose to get other documents and letters from us when you applied. You can view or change your communication preferences by signing in to your account.
If you chose:

Paper mail: We mail your forms to you when they are ready. An online copy is also saved in your Covered California account.

Online mailbox: You get an email or text message when your forms are ready in your Covered California account.
Note: You may get more than one form. This can happen if someone changed health plans or benefit levels (like switching from a Silver plan to a Gold plan), or if members of your household enrolled in different plans.
Didn’t get your IRS Form 1095-A or Form FTB 3895?
You may not get these forms if you were enrolled in:

Employer health insurance through Covered California for Small Business (CCSB)

A minimum coverage plan (also known as a catastrophic plan)
