If You Didn’t Receive Your Tax Forms
Some reasons why you may not receive an IRS Form 1095-A or Form FTB 3895:
- You were enrolled in a minimum coverage plan (also known as catastrophic plan).
- You were enrolled in the Medi-Cal program.
- You were enrolled in employer health coverage through Covered California for Small Business (CCSB).
If the above reasons do not apply to you, please sign in to your CoveredCA.com account and go to your secured mailbox to see if your tax forms are available to download. If you still don’t see your tax forms, please contact the Covered California Service Center.
